RESUMO

Diagnostic ultrasound (US) scanners are generally evaluated using proprietary quality assurance (QA) phantoms, but their prohibitively high cost may prevent organizations to perform the necessary tests. This study aimed to develop a low-cost gel wax phantom with targets to determine the lateral and axial resolution and diameter of a hyperechoic target in an US scanner. The acoustic property (AP) of gel wax, which includes the speed of sound (cus), acoustic impedance (Z), and attenuation coefficient (µ), were determined for multiple transducers operating at 2.25, 5, 10, 15, and 30 MHz. These results were compared to the AP of soft tissue. Two polytetrafluoroethylene (PTFE) rectangular frames with holes separated by 5, 10, and 20 mm were constructed. Nylon filaments and stainless-steel disc (SS disc) (diameter = 16.8 mm) were threaded through the frames and suitably placed in gel wax to obtain orthogonal targets in the phantom. The target dimensions obtained from computerized tomography (CT) and US images of the phantom were compared for phantom validation. The average cus=1431.4 m/s, mass density ρ = 0.87 g/cm3, Z = 1.24 MRayls, and µ ranged from 0.7 to 0.98 dB/cm/MHz for gel wax at 22 °C. The US image measurement exhibited a maximum error in determining the diameter of the SS disc, resulting in a value of 18 mm instead of its actual value of 16.8 mm. The phantom volume decreased by 1.8% in 62 weeks. The present phantom is affordable, stable, customizable, and can be used to evaluate diagnostic US scanners across multiple centers.

PURPOSE: The present study aims to design and fabricate a novel, versatile, and cost-effective Polymethyl Methacrylate (PMMA) head phantom for the dosimetric pretreatment verification of radiotherapy (RT) treatment plans. MATERIALS AND METHODS: The head phantom designing involves slice-wise modeling of an adult head using PMMA. The phantom has provisions to hold detectors such as ionization chambers of different sizes, Gafchromic films, gel dosimeter, and optically stimulated luminescence dosimeter. For the point dose verification purpose, 15 volumetric modulated arc therapy patient plans were selected, and doses were measured using a CC13 ionization chamber. The percentage gamma passing rate was calculated for acceptance criteria 3%/3 mm and 2%/2 mm using OmniPro I'mRT film QA software, and Gafchromic EBT3 films were used for 2D planar dose verification. RESULTS: Treatment planning system calculated, and the measured point doses showed a percentage deviation ranged from 0.26 to 1.92. The planar dose fluence measurements, for set acceptance criteria of 3%/3 mm and 2%/2 mm, percentages of points having gamma value <1 were in the range of 99.17 ± 0.25 to 99.88 ± 0.15 and 93.16 ± 0.38 to 98.89 ± 0.23, respectively. Measured dose verification indices were within the acceptable limit. CONCLUSIONS: The dosimetric study reveals that head phantom can be used for routine pretreatment verification for the cranial RT, especially for stereotactic radiosurgery/RT as a part of patient-specific quality assurance. The presently fabricated and validated phantom is novel, versatile, and cost-effective, and many institutes can afford it.

Materials with acoustic properties similar to soft-tissue are essential as tissue-mimicking materials (TMMs) for diagnostic ultrasound (US). The velocity (cus), acoustic impedance (AI) and attenuation coefficient of US (µ) in a material collectively define its acoustic property. In this work, the acoustic properties of polychloroprene rubber, beeswax, and Carbomer-gel are determined. The pulse-echo technique is used to estimate cus and µ. The product of a sample density (ρ) and cus gives its AI. Using a reference based on the International Commission on Radiation Units and Measurements Report-61, Tissue Substitutes, Phantoms and Computational Modelling in Medical Ultrasound, the results are evaluated. The acceptance criteria are 1.043 ± 0.021 g/cm3 (ρ), 1561 ± 31.22 m/s (cus), 1.63 ± 0.065 MRayls (AI) and µ within 0.5-0.7 dB/cm/MHz. Sample computerized tomography (CT) and US scanning are performed to evaluate their similarities (contrast and speckle pattern) with respective images of the human liver (a clinical soft-tissue). The average errors in measuring cus and µ were 0.14% and 1.2% respectively. From the present findings, acoustic properties of polychloroprene and beeswax are unacceptable. However, the results of Carbomer-gel ρ = 1.03 g/cm3, cus = 1567 m/s, AI = 1.61 MRayls are satisfactory and µ = 0.73 dB/cm/MHz, is higher than the reference (4.3%). Carbomer-gel could produce CT and US images, efficiently mimicking the respective liver images. Carbomer-gel containing 95% water is a low-cost material with a simple formulation. Present results suggest, Carbomer- gel mimics soft-tissue and can be used as a TMM for diagnostic US.

More often the embolic materials in the brain create artefacts in the planning CT images that could lead to a dose variation in planned and delivered dose. The aim of the study was to evaluate the dosimetric effect of artefacts generated by the Onyx™ embolization material during Stereotactic Radiosurgery/Radiotherapy (SRS/SRT) planning. An in-house made novel Polymethyl Methacrylate (PMMA) head phantom (specially designed for SRS/SRT plans) was used for this purpose. For the evaluation process, we have created concentric ring structures around the central Onyx materials on both the CT sets (with and without Onyx material). The verification plans were generated using different algorithms namely Analytical Anisotropic Algorithm (AAA), Acuros XB and Monaco based Monte Carlo on both CT sets. Mean integral dose over the region of interest were calculated in both CT sets. The dosimetric results shows, due to the presence of Onyx material, relative variation in mean integral dose to the proximal structure (Ring 1) were -4.02%, -2.98%, and -2.49% for Monte Carlo, Acuros XB, and AAA respectively. Observed variations are attributed to the presence of artefacts due to Onyx material. Artefacts influence the accuracy of dose calculation during the planning. All the calculation algorithms are not equally capable to account such variations. Special cares are to be taken while choosing the calculation algorithms as it impacts the results of treatment outcome.

Gafchromic films are increasingly being used for dosimetry in medical and industrial applications of ionizing radiation because of their favorable characteristics such as self developing in nature, easy to handle, dose rate independent response, insensitivity to normal room lights, high resolution and insensitivity to variation in the environment conditions. The dosimetry measurements using Gafchromic EBT film, Fricke dosimeter and thermoluminescence (TL) dosimeter were carried out on gamma blood irradiators to establish the suitability of Gafchromic EBT film in such applications. The dose rates determined by Gafchromic EBT film, Fricke dosimeter and TL dosimeter powder are found in good agreement to one another within the uncertainty of measurement. The two-dimensional dose distributions determined using Gafchromic EBT film reflects the expected dose distribution inside the sample compartment of the blood irradiator. The results of this study establish Gafchromic EBT film a suitable dosimeter for routine dosimetry on gamma blood irradiators.

A novel IMRT phantom was designed and fabricated using Acrylonitrile Butadiene Styrene (ABS) plastic. Physical properties of ABS plastic related to radiation interaction and dosimetry were compared with commonly available phantom materials for dose measurements in radiotherapy. The ABS IMRT phantom has provisions to hold various types of detectors such as ion chambers, radiographic/radiochromic films, TLDs, MOSFETs, and gel dosimeters. The measurements related to pre-treatment dose verification in IMRT of carcinoma prostate were carried out using ABS and Scanditronics-Wellhoffer RW3 IMRT phantoms for five different cases. Point dose data were acquired using ionization chamber and TLD discs while Gafchromic EBT and radiographic EDR2 films were used for generating 2-D dose distributions. Treatment planning system (TPS) calculated and measured doses in ABS plastic and RW3 IMRT phantom were in agreement within +/-2%. The dose values at a point in a given patient acquired using ABS and RW3 phantoms were found comparable within 1%. Fluence maps and dose distributions of these patients generated by TPS and measured in ABS IMRT phantom were also found comparable both numerically and spatially. This study indicates that ABS plastic IMRT phantom is a tissue equivalent phantom and dosimetrically it is similar to solid/plastic water IMRT phantoms. Though this material is demonstrated for IMRT dose verification but it can be used as a tissue equivalent phantom material for other dosimetry purposes in radiotherapy.

Medical cyclotron is a particle accelerator used in producing short lived radiotracers such as (18)F, (11)C, (15)O, (13)N etc. These radiotracers are labeled with suitable pharmaceuticals for use to gather information related to metabolic activity of the cell using Positron Emission Tomography (PET) scan. Target foil rupture is considered one of the major emergency situations during medical cyclotron operations because there is a potential of over exposure to the working personnel. Radiation protection survey of a self-shielded medical cyclotron installation was carried out during normal and emergency conditions. It is found that the induced activity in the target foil increases with its successive usages. As a case study, we have evaluated the emergency handling procedures of GE PETtrace-6 medical cyclotron. Recommendations have also been made to reduce personal exposure while handling the target foil rupture condition such as the use of L-Bench near the target area and participation of experienced personnel.

Orthogonal film-based treatment planning is the most commonly adopted standard practice of treatment planning for cancer of the uterine cervix using high dose rate brachytherapy (HDR). This study aims at examining the variation in rectal and bladder doses when the same set of orthogonal films was given to different observers. Five physicists were given 35 pairs of orthogonal films obtained from patients who had undergone HDR brachytherapy. They were given the same instructions and asked to plan the case assuming the tumor was centrally placed, using the treatment-planning system, PLATO BPS V13.2. A statistically significant difference was observed in the average rectal (F = 3.407, P = 0.01) and bladder (F = 3.284, P = 0.013) doses and the volumes enclosed by the 100% isodose curve (P < 0.01) obtained by each observer. These variations may be attributed to the differences in the reconstruction of applicators, the selection of source positions in ovoids and the intrauterine (IU) tube, and the differences in the selection of points especially for the rectum, from lateral radiographs. These variations in planning seen within a department can be avoided if a particular source pattern is followed in the intrauterine tube, unless a specific situation demands a change. Variations in the selection of rectal points can be ruled out if the posterior vaginal surface is clearly seen.
